Immunization Information:

• Each child entering a GA public (or private) school or daycare has to have immunization forms completed upon registration.  

• For daycare, the form is called a 3231 and has all the vaccinations a child has received on it to date.  

• For children ages 4 and above, a 3231 must be completed as well as a 3300.  A 3300 is a hearing, vision, dental and nutrition screening.  Both 3231 and 3300 must be completed for any age child if entering a GA school for the first time.

• It is almost impossible to get an appointment with a pediatrician during the summer since most parents make appointments in advance because kids are home for the summer.

• An alternative to the pediatrician office is a county health department.  Both of the forms can be completed in a health department, but there is usually a long wait (many parents doing the same thing) and can be challenging in other ways.

• Costs for form completion at the health department vary and are dependent on many factors.  If a child needs a vaccination, the health department is able to administer vaccines.
